What To Expect At Your Appointment

In order to provide a safe and comfortable environment for our patients & staff, we have updated our patient check-in process and visit for everyone’s safety during this time.

Before Your Appointment

1. To ensure proper authorization of your services, please send your vision or medical insurance information to our office prior to your visit. 

2. To complete or update your medical history and records, log in to your Patient Portal (instructions will be sent via Email). If you encounter log-in issues, please come into the office a few minutes prior to your appointment so that one of our staff members can further assist you.

3. Fill out the brief Wellness Form, a COVID-19 screening questionnaire.

4. Please wear a face covering during the entirety of your visit. 

During Your Appointment

1. We continue to maintain a modified patient schedule, allowing for social distancing and sterilization of equipment between appointments.

2. Patients are encouraged to come to their visits alone unless the assistance of a parent/adult is required.

3. Our optical dispensary is open. All frames are thoroughly sanitized according to CDC protocol and our optical staff will help assist patients at all times when handling frames.
